When the patient arrives in the ER

My Important Information
is an important tool to increase efficiency, improve patient care, and deliver more focused services to patients.

When a patient arrives by medical transport or car:

In addition to checking to medical alert pendant, bracelet, or other ID, check the wallet for the My Important Information wallet ID.

Note:  If the patient arrives by medical transport, the EMT or paramedic should present either the wallet card or the "refrigerator" card.

On your smartphone, scan the QR code.  It will take you immediately to the "First Responder Report." The screen displays selected information needed by first responders.  Select the green links to see more detailed information.

Note: If a smart phone with QR reader is not available, go to and enter the Emergency Code.
